Do not prescribe opioids and benzodiazepines or opioids and gabapentinoids concurrently unless transitioning from on to the other (increased risk of unintentional overdose)
Avoid antipsychotics for treatment of delirium or dementia unless non-pharmacological interventions have been tried and failed and the patient is threatening to harm self or others

Avoid Nifedipine immediate release as there is a potential for hypotension and risk of myocardial ischemia and avoid Avoid Amiodarone as first line therapy for A FIB unless patient has heart failure or substantial LVH. It is effective for maintaining sinus rhythm but has greater toxicities than other antiarrhythmics used in atrial fib
