Job Description

PURPOSE OF POSITION

PURPOSE OF POSITION: Under the supervision of the Director of the Cancer Center, the Survivorship / Oncology Nurse Practitioner (SONP) will serve as an integral member of an interdisciplinary hospital team. Comprehensive survivorship care planning (CSP) is an essential component of quality cancer care; with physicians, the SONP coordinates care and provides advocacy for patients who have completed cancer therapy and help manage patients diagnosed with upper gastrointestinal tumors, liver and pancreatic malignancies, metastatic cancers, advanced colon cancers, and soft tissue (non-breast) / other high-risk targets. SONP provides a continuum of cancer healthcare for the medically underserved community of the South Bronx and facilitates timely access to clinical services and resources.

Duties & Responsibilities

  • The Survivorship / Oncology Nurse practitioner will organize and manage the Cancer Survivorship Program (CSP).
  • Help provide patient navigation into the CSP and will coordinate, provide, and document care provided at those sessions in line with the Commission on Cancer and American Society of Clinical Oncology guidelines.
  • Participate in coordination of the plan of care with the multidisciplinary team, promoting timely survivorship follow-up on treatment and supportive care recommendations.
  • Coordinate and help manage care in Surgical Oncology and Colorectal to facilitate and manage the cancer of patients diagnosed with upper gastrointestinal tumors, liver and pancreatic malignancies, metastatic cancers, advanced colon cancers, and soft tissue (non-breast)/ other high-risk targets. This will involve prioritizing patients who require immediate care, streamlining diagnostic interventions, conducting patient assessments/procedures, identifying patients eligible for research trials, and facilitating therapeutic plans during the pre-operative care, postoperative care, and survivorship care phases.
  • Provide assistance to the Cancer Center to expedite and provide patient navigation to patients with suspected cancer on imaging, high-risk patients, and those deemed lost to follow-up with the help of the navigation team.
  • Maintain a database for active oncology patients and those who have completed treatment. Includes data such as demographics, dates of treatment, treatment type, recurrence of disease, outcomes, etc.
  • Assist in Commission on Cancer-related accreditation requirements concerning the Cancer Survivorship Program. - Provide updated reports and outcomes of all efforts. Presents quarterly at Cancer Committee meetings.
  • Attend Tumor board meetings, Cancer Committee meetings, and meetings as assigned by the director. Attend training sessions to improve knowledge and educational skills.

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

Departmental Preference: Strongly prefer National Board Certification in the specialty in which you are assigned.
