Now Hiring: High Risk Nurse Practitioner

Complete Care Management is seeking a dedicated and experienced High Risk Nurse Practitioner to join our team at Harborage. This role focuses on providing medical services in subacute and long-term care settings, emphasizing quality-driven care, prevention of rehospitalizations, and comprehensive patient management.

Responsibilities:

  • Conduct thorough patient rounds, including assessments, diagnoses, and development of comprehensive care plans.
  • Document all consultations and progress notes in electronic health records (EHR).
  • Manage billing through the designated electronic health system.
  • Communicate effectively with the interdisciplinary team to ensure coordinated patient care.
  • Maintain regular and effective communication with patients' families and support systems.
  • Focus on managing complex medical comorbidities and preventing acute care rehospitalizations.
  • Attend facility meetings to collaborate with the interdisciplinary team.
  • Work in collaboration with Complete Care’s Chief Medical Officer.
  • Report to the Director of Palliative and Geriatric Medical Services.
  • Exhibit professionalism and represent Complete Care positively.

Working Conditions:

  • Work across assigned Complete Care facilities, including subacute and long-term care units.
  • Travel between assigned facilities as needed.
  • Full-time position (40 hours per week, Monday–Friday).
  • On-call responsibilities (to be discussed during the interview).

Qualifications:

  • Graduate of a master’s-level Nursing Program.
  • Active Advanced Practice Nurse (APN) License and Registered Nurse (RN) License in [State].
  • DEA, CDS, and BLS certifications.
  • Compliance with continuing education requirements (CEUs) to maintain licensure.
  • Experience with electronic health records (EHR).
  • Must pass a physical examination and provide evidence of freedom from communicable diseases per facility policies.
  • Must be in good physical and mental health.

Experience:

  • Minimum 2 years of experience as an APN (preferred).
  • Previous experience in post-acute care strongly preferred.
  • Preferred skills include IV insertion, G-tube replacements, wound care, and suture/staple removal.
  • Experience managing elderly patients with complex medical conditions.

Community Outreach:

  • Assist the facility in integrating with community resources and healthcare services to improve patient care.
  • Participate in community and facility educational programs.
